In the current ACPIPHP notify handler we always go directly for a
rescan of the parent bus if we get a device check notification for
a device that is not a bridge. However, this obviously is
overzealous if nothing really changes, because this way we may rescan
the whole PCI hierarchy pretty much in vain.
That happens on Alex Williamson's machine whose ACPI tables contain
device objects that are supposed to coresspond to PCIe root ports,
but those ports aren't physically present (or at least they aren't
visible in the PCI config space to us). The BIOS generates multiple
device check notifies for those objects during boot and for each of
them we go straight for the parent bus rescan, but the parent bus is
the root bus in this particular case. In consequence, we rescan the
whole PCI bus from the top several times in a row, which is
completely unnecessary, increases boot time by 50% (after previous
fixes) and generates excess dmesg output from the PCI subsystem.
Fix the problem by checking if we can find anything new in the
slot corresponding to the device we've got a device check notify
for and doing nothig if that's not the case.
The spec (ACPI 5.0, Section 5.6.6) appears to mandate this behavior,
as it says:
Device Check. Used to notify OSPM that the device either appeared
or disappeared. If the device has appeared, OSPM will re-enumerate
from the parent. If the device has disappeared, OSPM will
invalidate the state of the device. OSPM may optimize out
re-enumeration.
Therefore, according to the spec, we are free to do nothing if
nothing changes.

ACPI / hotplug / PCI: Use _OST to notify firmware about notify status
The spec suggests that we should use _OST to notify the platform
about the status of notifications it sends us, for example so that
it doesn't repeate a notification that has been handled already.
This turns out to help reduce the amount of diagnostic output from
the ACPIPHP subsystem and speed up boot on at least one system that
generates multiple device check notifies for PCIe devices on the root
bus during boot.

ACPI / hotplug / PCI: Avoid doing too much for spurious notifies
Sometimes we may get a spurious device check or bus check notify for
a hotplug device and in those cases we should avoid doing all of the
configuration work needed when something actually changes. To that
end, check the return value of pci_scan_slot() in enable_slot() and
bail out early if it is 0.
This turns out to help reduce the amount of diagnostic output from
the ACPIPHP subsystem and speed up boot on at least one system that
generates multiple device check notifies for PCIe devices on the root
bus during boot.

ACPICA: Fix for a Store->ArgX when ArgX contains a reference to a field.
This change fixes a problem where a Store operation to an ArgX object
that contained a reference to a field object did not complete the
automatic dereference and then write to the actual field object.
Instead, the object type of the field object was inadvertently changed
to match the type of the source operand. The new behavior will actually
write to the field object (buffer field or field unit), thus matching
the correct ACPI-defined behavior.

ACPI / hotplug / PCI: Don't trim devices before scanning the namespace
In acpiphp_bus_add() we first remove device objects corresponding to
the given handle and the ACPI namespace branch below it, which are
then re-created by acpi_bus_scan(). This used to be done to clean
up after surprise removals, but now we do the cleanup through
trim_stale_devices() which checks if the devices in question are
actually gone before removing them, so the device hierarchy trimming
in acpiphp_bus_add() is not necessary any more and, moreover, it may
lead to problems if it removes device objects corresponding to
devices that are actually present.
For this reason, remove the leftover acpiphp_bus_trim() from
acpiphp_bus_add().

The check if the queue is full and adding current to the wait queue of
pending msgsnd() operations (ss_add()) must be atomic.
Otherwise:
- the thread that performs msgsnd() finds a full queue and decides to
sleep.
- the thread that performs msgrcv() first reads all messages from the
queue and then sleeps, because the queue is empty.
- the msgrcv() calls do not perform any wakeups, because the msgsnd()
task has not yet called ss_add().
- then the msgsnd()-thread first calls ss_add() and then sleeps.
Net result: msgsnd() and msgrcv() both sleep forever.
Observed with msgctl08 from ltp with a preemptible kernel.
Fix: Call ipc_lock_object() before performing the check.
The patch also moves security_msg_queue_msgsnd() under ipc_lock_object:
- msgctl(IPC_SET) explicitely mentions that it tries to expunge any
pending operations that are not allowed anymore with the new
permissions. If security_msg_queue_msgsnd() is called without locks,
then there might be races.
- it makes the patch much simpler.

Merge lockref infrastructure code by me and Waiman Long.
I already merged some of the preparatory patches that didn't actually do
any semantic changes earlier, but this merges the actual _reason_ for
those preparatory patches.
The "lockref" structure is a combination "spinlock and reference count"
that allows optimized reference count accesses. In particular, it
guarantees that the reference count will be updated AS IF the spinlock
was held, but using atomic accesses that cover both the reference count
and the spinlock words, we can often do the update without actually
having to take the lock.
This allows us to avoid the nastiest cases of spinlock contention on
large machines under heavy pathname lookup loads. When updating the
dentry reference counts on a large system, we'll still end up with the
cache line bouncing around, but that's much less noticeable than
actually having to spin waiting for the lock.

[SCSI] sg: checking sdp->detached isn't protected when open
@detached is set under the protection of sg_index_lock. Without getting the
lock, new sfp will be added during sg removal and there is no chance for it
to be picked out. So check with sg_index_lock held in sg_add_sfp().

[SCSI] sg: use rwsem to solve race during exclusive open
A race condition may happen if two threads are both trying to open the same sg
with O_EXCL simultaneously. It's possible that they both find fsds list is
empty and get_exclude(sdp) returns 0, then they both call set_exclude() and
break out from wait_event_interruptible and resume open.
Now use rwsem to protect this process. Exclusive open gets write lock and
others get read lock. The lock will be held until file descriptor is closed.
This also leads 'exclude' only a status rather than a check mark.

[SCSI] scsi_debug: fix logical block provisioning support when unmap_alignment != 0
Commit b90ebc3d5c41c9164ae04efd2e4f8204c2a186f1 ("[SCSI] scsi_debug:
fix logical block provisioning support") fixed several issues with
logical block provisioning support, but it still doesn't properly fix
the cases when unmap_alignment > 0.
For example, load scsi_debug module with the following module parameters
and make all blocks mapped by filling the storage with zero.
The problem is in map_index_to_lba(), which should return the first
LBA which is corresponding to a given index of provisioning map
(map_storep).

[SCSI] scsi_debug: fix endianness bug in sdebug_build_parts()
With module parameter num_parts > 0, partition table is built on the
ramdisk storage when loading the driver. Unfortunately, there is an
endianness bug in sdebug_build_parts(). So the partition table is not
correctly initialized on big-endian systems.

Earlier IDC-TIME-EXTEND aen was a nop and ignored by driver.
We now have to handle the aen so that other protocol drivers can use time
extension during some loopback operations.
[SCSI] qla2xxx: Print some variables to hexadecimal string via %*phN format
The patch changes a specifier used to output some variables. Instead of using
stack for each byte the '%*ph[CN]' allows to take a one pointer and prints
entire buffer as a hexadecimal string with the separator ':' or ''.

Enabling the data router support by default
can increase performance in certain situations.
It is safe to do so and tolerated in LPAR and under z/VM

[SCSI] mpt3sas: Added a driver module parameter max_msix_vectors
Added a driver module parameter max_msix_vectors. Using this module parameter
the maximum number of MSI-X vectors could be set.
The number of MSI-X vectors used would be the minimum of MSI-X vectors
supported by the HBA, the number of CPU cores and the value set to
max_msix_vectors module parameter.
The default value of this module parameter is set to 8. The default value of
this parameter is set to 8 inorder to reduce the amount of memory required for
Reply Descriptor Post queue. This is because with the higher MSI-X vectors,
some times kernel is not able to allocate the requested amount of memory and
crash is observed. To overcome this problem, the default value is set to 8.

[SCSI] qla4xxx: Implementation of ACB configuration during Loopback for ISP8042
While loopback diagnostic is in progress, disable the ACB which resets
all the active connections to target. Disable ACB would filter out all
the DHCP multicast and broadcast packets which otherwise cause the
diagnostic test to take longer time to complete or failures in some
other cases.
[SCSI] mpt2sas: Fix for kernel panic when driver loads with HBA connected to non LUN 0 configured expander
With some enclosures when LUN 0 is not created but LUN 1 or LUN X is created
then SCSI scan procedure calls target_alloc, slave_alloc call back functions
for LUN 0 and slave_destory() for same LUN 0.
In these kind of cases within slave_destroy, pointer to scsi_target in
_sas_device structure is set to NULL, following which when slave_alloc for
LUN 1 is called then starget would not be set properly for this LUN.
So, scsi_target pointer pointing to NULL value would lead to a crash later
in the discovery procedure.
To solve this issue set the sas_device's scsi_target pointer to scsi_device's
scsi_target if it is NULL earlier in slave_alloc callback function.

[SCSI] mpt2sas: when Async scanning is enabled then while scanning, devices are removed but their transport layer entries are not removed
When Async scanning mode is enabled and device scanning is in progress then
devices should not be removed. But in actuality, devices are removed but
their transport layer entries are not removed. This causes error to add
the same device to the transport layer after host reset or diagnostic
reset.
So, in this patch, modified the code in such a way that device is not removed
when Async scanning mode is enabled and device scanning is in progress.

[SCSI] mpt2sas: Change in MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED notification methodology
The intent of this patch is to perform a graceful shutdown of target drives even if
volume doesn't exits. Changes done in this patch
1. Removed the check for the presence of volumes before sending down
MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED. Therefore, this RAID action
would be sent if the card is IR Firmware.
2. The MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED is sent even when the
system undergoes suspend (in addition to remove/shutdown which was already
present)

lockref: implement lockless reference count updates using cmpxchg()
Instead of taking the spinlock, the lockless versions atomically check
that the lock is not taken, and do the reference count update using a
cmpxchg() loop. This is semantically identical to doing the reference
count update protected by the lock, but avoids the "wait for lock"
contention that you get when accesses to the reference count are
contended.
Note that a "lockref" is absolutely _not_ equivalent to an atomic_t.
Even when the lockref reference counts are updated atomically with
cmpxchg, the fact that they also verify the state of the spinlock means
that the lockless updates can never happen while somebody else holds the
spinlock.
So while "lockref_put_or_lock()" looks a lot like just another name for
"atomic_dec_and_lock()", and both optimize to lockless updates, they are
fundamentally different: the decrement done by atomic_dec_and_lock() is
truly independent of any lock (as long as it doesn't decrement to zero),
so a locked region can still see the count change.
The lockref structure, in contrast, really is a *locked* reference
count. If you hold the spinlock, the reference count will be stable and
you can modify the reference count without using atomics, because even
the lockless updates will see and respect the state of the lock.
In order to enable the cmpxchg lockless code, the architecture needs to
do three things:
(1) Make sure that the "arch_spinlock_t" and an "unsigned int" can fit
in an aligned u64, and have a "cmpxchg()" implementation that works
on such a u64 data type.
(2) define a helper function to test for a spinlock being unlocked
("arch_spin_value_unlocked()")
(3) select the "ARCH_USE_CMPXCHG_LOCKREF" config variable in its
Kconfig file.
This enables it for x86-64 (but not 32-bit, we'd need to make sure
cmpxchg() turns into the proper cmpxchg8b in order to enable it for
32-bit mode).
They aren't very good to inline, since they already call external
functions (the spinlock code), and we're going to create rather more
complicated versions of them that can do the reference count updates
locklessly.
vfs: reimplement d_rcu_to_refcount() using lockref_get_or_lock()
This moves __d_rcu_to_refcount() from <linux/dcache.h> into fs/namei.c
and re-implements it using the lockref infrastructure instead. It also
adds a lot of comments about what is actually going on, because turning
a dentry that was looked up using RCU into a long-lived reference
counted entry is one of the more subtle parts of the rcu walk.
We also used to be _particularly_ subtle in unlazy_walk() where we
re-validate both the dentry and its parent using the same sequence
count. We used to do it by nesting the locks and then verifying the
sequence count just once.
That was silly, because nested locking is expensive, but the sequence
count check is not. So this just re-validates the dentry and the parent
separately, avoiding the nested locking, and making the lockref lookup
possible.

vfs: use lockref_get_not_zero() for optimistic lockless dget_parent()
A valid parent pointer is always going to have a non-zero reference
count, but if we look up the parent optimistically without locking, we
have to protect against the (very unlikely) race against renaming
changing the parent from under us.
We do that by using lockref_get_not_zero(), and then re-checking the
parent pointer after getting a valid reference.

This behaves like "lockref_get_not_zero()", but instead of doing nothing
if the count was zero, it returns with the lock held.
This allows callers to revalidate the lockref-protected data structure
if required even if the count was zero to begin with, and possibly
increment the count if it passes muster.
In particular, the dentry code wants this when it wants to turn an
RCU-protected dentry into a stable refcounted one: if the dentry count
it zero, but the sequence number still validates the dentry, we can take
a reference to it.

ACPI / LPSS: don't crash if a device has no MMIO resources
Intel LPSS devices that are enumerated from ACPI have both MMIO and IRQ
resources returned in their _CRS method. However, Apple Macbook Air with
Haswell has LPSS devices enumerated from PCI bus instead and _CRS method
returns only an interrupt number (but the device has _HID set that causes
the scan handler to match it).
The current ACPI / LPSS code sets pdata->dev_desc only when MMIO resource
is found for the device and in case of Macbook Air it is never found. That
leads to a NULL pointer dereference in register_device_clock().
Correct this by always setting the pdata->dev_desc.
